Request a Transcript by Mail

We are no longer able to accept transcripts over the Internet. If you would like to request a transcript by mail, please print this Transcript request form (pdf 45KB) and fill it out completely. Send the completed form along with a copy of your drivers license to the following address:

We must have a copy of your drivers license along with the form or we cannot process your request.
Transcripts will be sent out the second business day after we receive your request.
Please allow 7 business days for processing during peak seasons.
