DCCCD VA Check List

Veterans – New Student Never used GI Bill Benefits)

Students must provide:

• DD-214 Form
• Official Transcript (Military transcripts are mandatory)
• NOBE (Notice of Basic Eligibility) – Applicable for Guard/Reservist

Student must fill in during application process (forms available at VA Office):

• VA Form 22-1990
• Information Worksheet
• Degree Plan
• Standards of Progress
• Enrollment / Certification procedures

Veterans – Transfer Student (Has used GI Bill Benefits)

Students must provide:

• DD-214 Form
• Official Transcript (Military transcripts are mandatory)
• NOBE (Notice of Basic Eligibility) – Applicable for Guard/Reservist

• Degree Plan
• Standards of Progress
• Enrollment / Certification procedures

Dependents – New Students - Non-Military (Never used GI Bill Benefits)

Students must provide:

• DD-214 Form
• Birth Certificate*
• Official Transcript

*Students that have "Certificates of Eligibility" do not need to provide their birth certificates.

• Information Worksheet
• Degree Plan
• Standards of Progress Enrollment / Certification procedures

Dependents – Transfer Students -Non-Military (Have used GI Bill Benefits)

Students must provide:

• DD-214 Form
