2. Please ask for immunization record or any proof of receipt from the administering health agency for your school registration purposes
3. If you have Health Insurance, please take your card with you. Some agencies accept Health Insurance plans.

Guidelines for requesting an exemption for reasons of conscience

To obtain State Exemption from the Meningococcal and other vaccines, please visit the following state web site:
www.dshs.state.tx.us/immunize/school/default.shtm#exclusions

Once you complete and return the Exemption Request Form, an Affidavit Form request will be processed and mailed within one week from the receipt of the request.
• Email or telephone requests cannot be processed.
• The official Texas Department of State Health Services Affidavit Form must be notarized and submitted to Eastfield College officials. The form must be submitted within 90 days from the date it is notarized. Eastfield College will accept only official Affidavit Forms developed and issued by the Texas Department of State Health Services (DSHS), Immunization Branch. No other forms or reproductions will be allowed.
