Sign In Procedures:

1. Fill out your name. Last name, then First.
2. Fill out your I.D. number not your social security.
3. What course you are taking? English, Math, Sciences, Accounting, and Spanish all require a separate sheet.
4. Answer the two questions.
5. Then read the contract and sign and date.
6. Turn the sheet over.
7. This is your log sheet!
8. Each time you come in ask for this form.
9. Put the time and date on the form.
10. Keep the form with you while you are in the facility.
11. Have your tutor initial your form.
12. When done return the form to the front desk and place it in the appropriate box.
13. Remember the color of your form!
