All VA Students are required to submit the following documentation for certification. The documents are grouped in order of how they relate to the individual education programs.

General Documents Required for ALL VA Education Programs (Check the specific program you are applying for to identify additional required documents)

Registration summary (must be submitted every enrollment period)
Official Transcripts from all Colleges and Universities attended.
DCCCD Degree Plan Request
Official Degree Plan from the Admissions Office.
Student Contact Information Sheet / Check List (This form will be provided to the student by the FAVA office)
DCCCD VA Data Sheet
Direct Deposit Information (Bank Account Number / Routing Number

Chapter 30 Montgomery GI Bill – Active Duty Educational Assistant Program
